To submit a nomination to the National Science Board (NSB), please complete the form below. There are essentially three parts to this form; Nominator Contact Information, Nominee Attributes, and the Nomination Files upload section.
Nominator Contact Information is necessary in the event that the NSB administrator needs further information about the nominee.
Nominee Attributes are used to aid in the review process by helping the NSB administrator categorize and sort nominees by attribute.
Lastly, three files are requested to complete your nomination submission:
